My dog will only drink from the bowl if we hold the bowl up for her. She scratches and barks at us until we give in and hold her bowl up to her mouth.

I wouldn't give in, an animal or a person will eat or drink if they are hungry or thirsty, don't think they will starve themselves.
May need or want attention?

Solution is simple. Raise the bowl to the level she likes and then SLOWLY lower the bowl over time.

How old is she and how long has this been going on? Is there something that triggered this behavior or has she been trained to drink that way?

You might try getting an elevated bowl and 'pretend' to hold the bowl up for her until she gets used to it.

Would he drink out of his dish if it was in one of those elevaed stands. My dog max likes his food dish on a foot stool
He'll eat weather it is on the stand or on the floor, but it seems that he much prefers it to be elevated.

How large is the dog?
My parents Great Dane doesn't like eating or drinking out of bowls on the floor he has both bowls off the ground on special holders.
