Tile remodel about to install new toilet . The flange sets about 1/4" below tile/floor level is this a problem. Should I use some sort of extender or go with a taller wax ring

This shouldn't present a problem as the bowl gas a horn that extends below floor line. If you're concerned purchase a wax ring with a attached funnel. Good luck, Tom

Lower is better than Higher - and 1/4" should not be problem at all. As Tom advices don't be afraid to use 2 wax rings: one fat one with flange and skinny one on top of it.

I do tile and toilets when remodeling. I shoot for the flange to be flush with the floor or the bottom edge of the flange flush with the floor. You are below that but not by much. An oversized wax ring or a flange spacer would do the trick, never been a fan of trying to double up 2 wax rings so skip that
