Hi,
Im 3d artist from Czech Republic. I filled w-8ben here. To claim tax treaty benefits I need to obtain an ITIN number from IRS.
I know I must send:
1) W-7 form
2) notarized passport
3) I have document from turbosquid, where they write that I uploaded my models and may have some sales.

Document and passport I have, now I need help with filling this W-7 form

I'm nonresident alien required to obtain ITIN to claim tax treaty benefit,
So I fill a)

My question: need I to fill h) also? If yes what I must write do dotted line ? "exception 1"?
Treaty country is czech republic, but treaty article number? I really don't know :(

No need to fill in h).

Download IRS Pub 901 (Tax Treaties) from www.irs.gov to find the article you need.

And when I filled this H and wrote "exception 1" ?
Am I wrong? :( I already sent it :(

I downloaded pub 901 but I found nothing to czech republic, but maybe I only don't understand this

No, exception 1 is fine. If you filled in H, it should not cause a problem.

If you could find nothing for Czech Republic, that means no tax treaty exists.
