Hello,

My wife visa is expired. But, I have renewed I-94. While Filing W7 form, It requires VISA Type, Number and Expiration. What should I fill for VISA Type, Number and Expiration.

Please let me know.

Wife Visa is H4 and I am on H1B

Use the information on the renewed I-94 which shows the new expiration date.

Should I write VISA# as I-94#? Do I need to notarized expired visa and I-94 pages?

Yes, use the numbers on the I-94.

You do NOT need to notarize the I-94 pages, but attaching a copy of the I-94 to the W-7 (along with the notarized passport photocopy) is a good idea.

Since VISA page is expired, Should I notarize and attach it along with W7?

No. That is the reason you are sending the I-94, because the original visa is expired and the I-94 effectively replaces the visa until you can get a new visa issued.
