In ireland, it can be difficult to get the registered birth name changed.

It is easier to do for a child's first name, but much much more difficult to do for a child's surname.

If the child is commonly known since birth by another similar name, such as Carol Anne instead of Caroline, it can be done.

There are forms to be filled and evidence from professionals, i.e. doctors, to be supplied. The forms are available from GRO Ireland (http://www.groireland.ie) and from Civil Registration Offices (http://www.hse.ie/eng/Find_a_Service/BirthDeathsMarriages/Find_your_Civil_Registration_Office/)

Birth Certificates are available from these offices and online from Irish Certificates (http://www.irish-certificates.com/)
