First, try the employer; they are required to keep a copy of all W-2s for ten years.
Second, if you had the return done professionally, go to the tax professional. He has a copy of the W-2 on file.
Third, call 1-800-829-1040 and ask the IRS to mail or fax you a copy of your tax transcript. The FEDERAL tax information from the W-2 will be on that tax transcript.
Fourth, call the state tax department and ask for a copy of your state tax transcript; the W-2 should be on that transcript as well.
