View Full Version : Son named as benefactor on bank account

My wife's ex recently passed on and it became knowledge that he had named his son, my step son as a benefactor on his bank account. My family and I live in Pennsylvania while the ex husband lived in California. I contacted the union bank in San Diego and was told that I need death cert. birth cert. and declaration of legal guardianship form 01034. I can not locate this form any where. Asking help if this is the right form , and where to locate it , or if known which form is needed for the bank. THANK YOU

I use to work for a bank and I can tell you, most of the employees are morons. That form doesn't sound right. You should only need the death certificate and identification of the person who is the beneficiary. Though if that child is a minor then you may need a the form in which they may be referring to, to prove the guardian of the child. The bank should have a copy of the forms THEY require. If need be I would go into a local bank and ask what their proceeder is. Sounds legal and all banks should have the same proceeder.
