All Minors Age 16 and 17 shall:

Appear in person at the time of application
Have a parent or guardian present photo identification if the minor does not have identification of his or her own
Establish parental consent (You may be requested to provide written parental consent for a child age 16 or 17 if it is not implied on the application)

Special Requirements for All Minors Ages 16 & 17 (http://travel.state.gov/passport/get/minors/minors_4313.html)

Both parents should be signing the documents for a passport. Also if she intends on using it before she turns 18 then your going to need the fathers permission to leave the country.

Why hasn't paternity been established ?
