My Doctors' Office Manager released my files to a local Hospital without my written consent under the assumption that I had found a new Primary Care Physician, which I did not. All that I was that a referral be sent to this Hospitals' Ortho Clinic since I had lost my job and insurance and needed treatment for arthritis. I was sent the forms to sign for the release which are still in my possession and unsigned. What can I do? Also, this is not the first time this "manager" has made the wrong decision regarding my health.

First, its HIPAA, not hippa.

It sounds like they were trying to conform to your request, regardless of the release form you failed to present.

What is it that you want to happen?

Ask that the hospital send your records back to the doctor. I'm not sure this would qualify as a HIPAA violation. It appears like the office was trying to help you, but may have been overzealous
