How can I find samples of these documents needed to modify my decree in Nebraska (Sarpy County) to go to joint physical custody and eliminate child support (ex-wife agrees)? I'm going to write them myself and file with the court clerk:

Motion to Modify Divorce Decree
Court Order (to submit to judge for approval)

Most likely they will not be available in a fill in the blank sort of form.

If you can't afford an attorney, check out the legal aid society,
Also a local attorney can merely write the motions for you to file very cheaply

I don't know how it works in Nebraska but in California you don't have to modify the divorce decree, you merely modify the child custody orders. That only requires a run-of-the mill motion. Pretty easy to do here.

I want to file a motion to modify medical decisions and medical custody from my ex wife to me. She is in agreement

Where? In general one of the parties makes a motion, there's a hearing, that or those particular clauses are modified and/or changed.

The decree remains as it was, with a modification.
