Example scripts please.

The first number is the major correction. Negative if nearsighted and positive if farsighted. 1/number is the focal length in meters.

The other two numbers, if present, is the Astigmatism correction.

The weaker eye has the -0.25, or the one with the higher focal length (1/0.25) meters.

There are said to be 4 main visual problems myopia(shotsighted- can see fine for close work but long distances are blurred these types of prescriptions are written with a minus sign as the power of the eye is "too strong") hypermetropia (long sighted- can see into the distance but close objects are blurred these are written with a plus sign as the eye is said to be "too weak") emmetropia ("perfect sight" and I use the term quite loosly) and astigmatic (the eye can't focus light altogeather i.e. half is in focus and the other half not- this is the second part that can sometimes be in your prescription and can be written in either plus or minus) eveyone at some point will need a reading add to help them in later life
