Welcome to AMHD. You need a lawyer.

What state are you in and do you have any existing orders from the court concerning this matter ? What exactly are you trying to accomplish ?

What he said. I'm not sure OP knows exactly what he/she is asking -

This is a hard issue, is the other party in agreement ? I recently did one and it has to include a parenting plan written up from a state specific forms. When I was done the child custody agreement was over a inch thick. This is not a simple issue, unless both parties agree. Even then I would use at least a paralegal

I think he's looking for the format of a Motion - that's why I'm not sure what he's asking. Also what State. Varies State to State. Some use forms, others use cut and paste documents.

Doesn't sound like a separation agreement/custody agreement. Of course, I've been wrong before - :)
