I am looking to find a letter template to use to request to a creditor a detailed account history showing debit, credits, payments posted etc. can anyone hook me up to a letter template or link

Why do you need something formal?

I am requesting a complete history of my account <insert account #> showing all activity from <start date> to present. Please forward this history as soon as possible.
