I would like to work at a nursing home, but I am to afraid to show that I have a record on my police check. It is theft under $5000, and it happened a year ago. It was a stupid mistake, I was 15 years old... "stole" a chocolate bar... except I never took it out of the store.. and then I got a warning from the police, but they said they wouldn't put it on my record... however they did.
Am I allowed to work at a nursing home with a record? Will they kick me out once they see that?

You definitely will not be able to work at a nursing home with a theft record for sure. Any nursing home worth its salt would require a police check before hiring. In Canada one has to be a certified Personal Support Worker to work in a nursing home and that requires somewhat more then a police check.

This is a misdemeanor and should not effect it from a legal standpoint, Now the home may be concerned that you may steal something from the patients but that would be a management decission.
All you can do is go and apply and apply to several.

Sitting and worrying what they may do will never get anything done
