I have to do a 5 point care plan for a patient in my care. My patient is female, 47yrs, lives alone. Was admitted into hospital with fractured tibia, sustained following a fall whilst under the influence of alcohol. She did not seek help for 10 days and may lose her foot. She did not relalise it was fractured. She has had surgery for the fracture, external fixator, the wound is open, needing regular debridement. She suffers from depression following divorce, takes reguar medication. She has a sacral sore and is constipated.
At the moment I have mental block and need inspiration

catlov, you may have mental block but I have to do careplans on my own with no one to ask and I wouldn't ask it here. Your own thoughts and experience with the patient, observing, knowing her meds first hand, viewing her as she lies, sits, or as we say in health care, ROM.

I just hope she gets proper diagnostic evaluation in regard to alcohol. You need some hereditary info in regard to liver, kidneys, (diabetes and scirosis). You don't say where the pressure ulcer is, to us could be anywhere and where it is may be important to her health (in certain areas of the body they are extremely difficult to eliminate). And, lastly, but no less important, the poor lady is constipated.

So sorry, never used this site before, not really done a care plan before.
