I posted this once before and am quoting myself here:

"It must be in writing, specifically typed, and it must be filed (not simply mailed) with the Clerk of the Court where the matter was sued and a copy furnished to the Attorney or agent for the creditor and it must be TYPED, SIGNED IN THE PRESENCE OF A NOTARY AND NOTARIZED.

You use the same heading they used - names, Courts, index number and so forth. The wording in the body is: ‘(1) I, John Jones, deny that this is my debt; (2) If this is my debt, I, John Jones, deny that it is still valid; (3) If it is still valid I, John Jones, deny that the amount in suit is the correct amount of the debt.’

Note that the wording should not be any more specific. The advantage of a sworn denial is that it requires the creditor to produce a witness to the debt and a reply Affidavit (from the creditor’s Attorney or agent) is not sufficient.”
