I worked and lived in NY for 8 months and worked and lived in NJ for 4 months; which state income tax forms do I need to fill out?

You will file returns for both the states as part-year resident.
For NY -- IT 203 Nonresident or part-year resident return
For NJ - 1040NR Nonresident tax return.
