When filing taxes with a 1040A form, do you include the 401k lump sum payout with the wages earned for the year?

Depends if you owe a 10% penalty for early withdrawal. If you owe a 10% penalty you must file a form 1040, and on that form you show your distribution on line 16 ("Pensions and Annuities") and the 10% penalty on line 60. If you don't owe the penalty (i.e. you are over 59-/12 or meet one of the other penalty exceptions) then you can use form 1040A - list the distribution on line 12. You should have received a 1099-R form from the plan, showing your withdrawal as well as any taxes already withheld. There should also be a distribution reason code shown on the 1099-R; if you tell us what that code is we can confirm whether you owe the 10% penalty.
Finally, please confirm that you did not roll the distribution over to an IRA.
