It is a 1099 (Rev.9-07) form. I completed and submitted the form 1040 for the tax refund last year. I received the tax refund in a bank account, now I received this form. I don't know many details about tax forms, as I am not an US citizen, nor an US permanent resident. Please, make it clear.

Alina - did you see my response to your earlier post? Is it still unclear? Please confirm that what you received is a 1099-G form. It is most likely that this is a refund of state income tax you previously paid - did you file any state or local income tax forms for 2006 (or earlier)?

I received a similar form (1099 Rev 9-07) too. Its NOT a 1099-INT or 1099-G.
The form has just 5 unnumbered boxes and it shows the interest credited on the overpayment of federal income tax in the year 2007.
I am a non-resident alien and a student here in the US. I am not sure how should I report this income.
My Questions?
1) Should I report this as income?
The form I received says that I should.
2) Do I file 1040NR or 104NREZ? If its NREZ, where do I find instructions (that are readable on a lazy afternoon... )
The amount is just a $15. I'll be damned if I have to fill up form 1040NR :(.
