View Full Version : J-1 waiver without a copy of DS-2019 form possible?

Please help me!

My employer is going to sponsor my H-1B visa, but I just find out that I have to get a J-1 waiver before I apply for H1B. However, I don't have my original DS-2019 form with me! What should I do?

I was on J-1 visa on 2004 for a summer ESL program, but then I transferred to another US university on a F-1 visa. No one ever mentioned anything about getting a waiver, so I no longer have my DS-2019 form.

I've been trying hard to find a contact info of the J-a waiver program, but all my efforts are in vain. I have contacted my former university at Michigan. They no longer have my DS-2019 on file and gave me a sevis record where I was able to find out the program number , the program code and start date, end date. However, after submitting the online form, I have to mail a copy of my DS-2019 form to a J-1 waiver office.

Hey suzee,

Did you find an answer to your question? I'm in the same case and the only thing I'm missing for my visa is this DS-2019 form (which I no longer have.. )

This would help greatly if you could tell me how you did ! Thanks in advance !

Did you really want to ask suzee? Her last post on this forum was about three years ago. There are several active immigration experts on this forum. Good luck.

I actually have a J1 visa. I'm also applying for a waiver. They require all copies of my previous DS 2019! I can't find them all. Anybody can help?
