How do I go about adding my last name to my child's farthers last name for her to have both last names?

As far as I know you can call your child whatever you like.

How do I go about adding my last name to my child's whom has her father's last name on his/her birth certificate? Can the child have both last names. Can the mother's last name just be added on to the birth certificate?
