It's great that you want to take care of your health at such a young age, and when you start this young taking care of yourself, and watching what you eat, you'll have a easy time of it the rest of your lives.
You all really don't need a "diet plan", just make healthy food choices! Instead of junk food, or high salt snacks, snack on fruit, try to keep the amount of sugar you eat low, things like soda contain a lot of sugar and no real nutrition your body can use. Try not to eat too much fast food, fried foods, there's a lot of healthy foods out there with out all the excess fat and salt that you don't need. Small amounts of lean meats like skinless chicken, or turkey breast, fish, loin type meats, instead of processed meats, like sausage, bacon, process lunch meats, You need dairy every day like lowfat or skim milk, cheese and yogurt, to help keep your bones strong. Drink water or 100% fruit drinks, instead of presweetened drinks or soda. Fruits and veggies with everymeal, snack on fruits, small amounts of nuts and seeds, these can be added to your yogurt. Make sure you eat a really good breakfast every day, it's really important to start your day with something like a whole grain cereal, or oatmeal, wheat bread, fruit. Look into what is in the foods your eating, start reading the nutritional labels that are on most foods, it takes a little while and you may not understand everything but it's good to find out what's in what your eating. Another way to learn more is go to: The Food Guide Pyramid (http://www.kidshealth.org/kid/stay_healthy/food/pyramid.html). This site will teach you a lot about what you need to eat and it's really interesting to read!
