I need to find out where my daughter can get a free form for her husband ro relinquish his parental rights in Texas. Already filled out divorce papers and the divorce is uncontested. Need for husband to give up rights, he is schizophrinic with multiple personality disorder and depression. Please help.

Have your daughter call the local courthouse in her area or consult an attorney.

Normally there is no fill in the blank forms, the attorney who is doing the divorce could request this, but seldom do judges grant it, plus since it does not stop the person from having to still pay child support, most parents won't agree.
But as part of the divorce with children, there has to be child custody agreements filed, in those filing, the father can give up all visitations and the such if he will agree.

I don't know if any such "form" exists. Also she really can't compel him to "give up his rights" due to his mental condition. If his condition is in fact as serious as you suggest then his rights would be severely curtailed regarding visitation and the like and he'd certainly never be able to get custody but he'd never have his rights completely taken away unless you could demonstrate that he is a danger to the child(ren.) Even then it wouldn't necessarily be permanent as his condition may well be treatable.

I hope your daughter has an Attorney because if her husband has mental problems and does not contest the divorce at this time he can always come back and say he did not understand, could not comprehend the divorce, had diminished capacity and attempt to reopen the divorce proceeding.
The Court might suspend visitation but she cannot force him to give up his rights.
