Uhhhh, not really, and maybe. I know, that doesn't answer your question. But, legal questions AREN'T simple.
Each state has its own landlord tenant laws. As a landlord, you already should be familiar with yours. Actually, they're the RULES of the landlord business, and that's the business you're in. You can find a copy of your states laws at the top of this forum on a sticky note.
Once you read the law on the subject, you'll know how to frame your notices.

There really isn't any template. The key is what you want to do about the break. If you want to evict her, then check the rules for eviction in your area and start that process. Generally, it starts with a written statement given the tenant x number of days (usually 3-7) to vacate or comply. If the tenant hasn't done either after the specified time, you file with the courts for an eviction order. This usually involves a hearing. Once the coirt issues the order, you need to hire a sheriff to serve the order and oversee the eviction.
