I am an RDA at a dentist office and I am leaving next week to start at another office. I have worked at the current place for 5 years and have very good relationships with my patients. Many ask specifically for me when scheduling appointments because they really like me a lot and trust me to take care of them when they are afraid. I wanted to write a few of them goodbye cards, to let them know I wasn't just deserting them without a thought. Is this OK to do?

It is okay to do however do not tell them in the cards where you are going. That could be construed as you attempting to "steal" patients. When I left my last job I had formed very tight relationships with some of our vendors and recruiters. When I left I sent emails to everyone I worked with letting them know how much I enjoyed our relationship while I worked at X company. I did it while I was still an active employee so again there is no look that you are attempting to take anyone with you.
