Do you put food and/or water in crate during day if you need to be gone about four hours?

No, I leave nothing in the crate except a sturdy chew toy, most often a Nylabone. At 14 weeks, a healthy puppy should do fine for 4 hours. I never suggest leaving any dog more than 5 hours in a crate, so 4 hours is fine.
