I did not receive my 1099-R form for 2016 taxes. How do I get it?

Contact the company that was to send it to you. They are mailed out by 1/31 each year. So the company may not have mailed them yet
So who you worked for, must provide it.

They all wait until the last minute to mail them, so do not get antsy until this time next week.
