I have been in the U.S. since August 2001 as a student (one year on a J1, subsequently on a F1 visa). I presume I am considered a resident alien for tax purposes. However, I have received the following forms:

(1) W2 from university: wages (teaching/grading). Taxes were withheld.
(2) 1042-S from university: fellowship. This form indicates that no taxes have been withheld, because of a tax treaty with my home country. However, the treaty indicates that after 5 years, the fellowship is taxable income.
(3) 1042-S from a company for a financial award. Taxes were withheld.

Should I file a Form 1040 and attach the two 1042-S Forms in addition to the W2? If yes, on which line should I write down the taxes withheld on my financial award (3)?

Thank you in advance for your help. I hope I have provided enough information.

Choi:

You will NOT be able to file this return electronically, because there is no specific place to put the data from the Form 1042-S.

You can enter the income portion either on LIne 7 with WAGES or on Line 21 (OTHER INCOME).

Add the withheld income on the same line where the WITHHELD TAXES from the W-2 go.

You can enter the Form 1042-S income on the W-2 forms in the tax software, but you CANNOT efile. Just print off the tax forms, attach the Forms 1042-S to the return, and mail it. Same for the state tax returns.
