2nd floor bath in 100+ year old home. Having problems figuring out what the right way is to vent? Can't get directly up the wall behind the toilet. Does the vent have to be above the tank.

This is a typical bathroom vent using the lavatory as a wet vent. Why are you concerned with venting at this time? Back in Wisconsin we have century old homes with one 4" vent for the entire house. All fixtures had deep "S" traps. Sounds like you have the same thing. Why do you want to start changing things? Are your other fixtures vented? Back to you. Tom
