Our bathroom has a constant urine smell. It is kept clean, but the smell is still there. Any ideas?

Possibly an odor from urine that may have run down the side of the toilet and gotten underneath the toilet bowl where you can't clean it...;) Here, you can try caulking around the base of the toilet with silicone and see if that resolves the issue.
It could also be the wax gasket is letting sewer gasses get by, but in most of those cases you'll usually see some water near the base of the toilet.
Try caulking around the base of the toilet...

Also check the area around the lid at the back. Easy for urine to "hide" there and create problems.

Mark, I'd worry that caulking around the base could hide a bad wax ring if it were to fail. You could get the subfloor soaked. For $8 you could become a detective and find the urine. Good point, Bob...should have said to caulk around the base of the toilet EXCEPT the rear 3-4"...should eliminate odor and still allow a bad gasket leak to show up before any real damage can occur.

From my experience, it is urine that failed to hit the target or urine that is not completely flushed.
