My girlfriend regularly has her menstrual period between the 25th of each month and the first week of the next month. Her last period started in September 30. We had unprotected intercourse at Oct. 4, her period ended Oct. 5. I just used the withdrawal method and though my ejaculation happened outside of her, I would just like to know if there is a chance of her getting pregnant?

Yes. Low odds, but the chance is ALWAYS present when two people have sex, regardless of when or what protection is used. Withdrawal is not good birth control.
And no woman has her period at the same time each month forever. She changes over time, with her health, with stress, with all sorts of changes.

Ovulation is always at the 14th day after the first day of the period right?
No. Ovulation is not always the 14th day. Ovulation can change for many reasons. General healt, stress, illness, weight gain/loss.
Rarely does ovulation occur on the same day every month. Any time a penis is inserted into a vagina pregnancy is a possibility.

Any time there is penile insertion there is the risk of pregnancy. You can reduce the risk, but not eliminate it. Ovulation is not that predictable. It also takes time for sperm to travel. So conception may occur days after sperm is deposited.
As noted the possibility is minimal, but not impossible.
