If you're going to diagnose someone's dog on the internet, you'll have to provide the exact info for your diagnosis, and then you should be prepared to back up your words when you're wrong.
So, how did you come to the conclusion that this dog will die if it doesn't go to a vet? Not that I disagree with the OP taking the dog to the vet, that's the right thing to do, but how do you know it will die? What blood work have you done, what in the OP's post lead you to this conclusion? What disease do you think the dog is suffering from?
You have to specific in this forum, this is a fact forum, not an opinion one like the relationship forum. Here you can't guess, it's not allowed.
So please, post your facts.

How long has he been lethargic? Has he eaten anything unusual today? Is there a possibility that he got into something he shouldn't have, like garbage, bones, etc? Is he urinating and defecating normally?
If you feel that he's ill then a vet really is your only option. The vet will ask you the same questions I just did, and more. If he ate a bone, or got into something he shouldn't have, he could have a blockage. If he's defecating (pooing) normally, then this is unlikely, but not impossible.
It sounds like he got into something he shouldn't have, and it's causing him to feel ill. If you don't know what he got into, then the vet really is your only answer.
Good luck.

Yes, and that was your puppy, not the posters puppy. There's more than one thing that can be wrong with a dog that's feeling ill. So, unless you know for sure (by doing blood work and tests on the OP's dog) you can't diagnose the dog with the same thing your puppy had. Even then you couldn't because you're not a vet.
You can't just guess on this forum, you have to stick to facts, and the fact is, your dog and the OP's dog are two different animals, and you can't assume they have the same issue.
It's the same as you telling someone "you have the flu because that's what happened to me", without knowing what's wrong with them. Just because you had the flu once doesn't mean they do. Do you see what I'm saying?
