I want to know where to go to get the eviction notice papers from.

It's NY law, not specific to any City. This is a good site - New York Evictions (http://www.rentlaw.com/eviction/newyorkeviction.htm) - and the forms are available.

You get the eviction papers from a court, but the 30 day notice is not an eviction. Its just the first step in the process. You only need tell them you are terminating their tenancy at the end of 30 days.

If they don't vacate, then you go to court to file for an eviction.
