My husband and I(F1/F2) arrived in U.S. August,2006. This year is our first year to file tax return as a resident alien for tax purpose. We decide to file 1040 jointly so I have to apply a ITIN. I have some questions about how to fill in W-7 form.
First, my F2 visa in my passport has been expired. But My husband has renewed his F1 visa because He went out of US for interviews alone this March. How should I fill in '6c: Type of U.S. Visa,number,and expiration date'?
Second, should I provide my passport and I-20(issued by school) together?
Third, in '6d:Entry date in U.S.' Should I put the first date I came here(2006) or last time I came here(2010)?

Enter the expired F2 visa info in Box 6c. As long as the passport is not expired, you will not have any problems.

Put in the latest entry date.
