List your volunteer work with dates of service.

I second what Wondergirl said... you wouldn't put down that you DIDN'T do something... you only put down what you did. That was meant more as a guide to what sorts of information if they apply that you should put down there. Many people would not have anything to write there for example.

May be good for us to know your age, and the position your applying for.

Definitely put down volunteer work.

You can have a little bit of fun on the application. For instance, when I applied to a grocery store at 14, and under extra-curricular activities, I put down "I play in a metal band", which, turned the interview into a conversation about music and definitely helped in getting the job. Stuff like that shows creativity, confidence, and interpersonal skills.

Interviews are one of the best times to practice interpersonal skills.

Yes, list any school clubs and special projects (science fair winner, etc.), any church groups, peer tutoring, Scouts, 4-H, social service-community efforts--anything that shows you off as a wonderful, helpful, organized person.
