View Full Version : What if you never had a birth certificate only a midwife document of birth.

I was delivery by a midwife in 1966. My mother was given a document of birth but did not turn it in to vital office as an adult I used the document as a birth certificate and it was accept but now it was destroyed and that was the only copy. The midwife died a few years ago. I am try to get my driver's license. What can I do?

You will need to contact the vital records department of our state ( if you are in the US) and see what is required. Do you have a social security card ( if in US) You are over 40 and never had a drivers license?

Obviously I cannot tell you exactly what you will need to do without at the very least knowing what state or country in which you were born.

Contact the department of vital statistics in your state and ask them. They will probably have some procedure for obtaining a substitute birth certificate. I would expect that you would be asked to find one or more persons who know the circumstances and who can sign affidavits as to what they know.
