Submersible pump 60/40 pressure gage,set at 38 lbs system holds pressure with no water running, after a week no water pressure and bladder is at 0, drained systerm and bladder has no water, re-charger bladder every thing is fine for about a week, bad tank?

A few observations

1. The bladder should never lose pressure, even when the system has been drained and no water is in the tank. That indicates the bladder in your tank is shot, or the air valve at the top is possibly leaking (possible, but unlikely). I suspect that when you believe you are recharging the bladder, you are simply pressurizing the tank. You won't get water out of the air valve (assuming ruptured bladder) until several weeks have gone by and the air at the top of the tank has dissolved into the water, thus allowing the tank to be largely filled with air.

2. You have described two problems. The first concerns your system losing pressure over a week's time. That indicates a leak, either in the well, or towards the house.

3. The other problem concerns why your pump is not cutting back on, even though pressure has dropped to zero.

So, to help us out a little, how do you restart the pump when you discover the pressure is at zero? Does your switch have a reset valve? Also, do you have a valve to cut off water to the house? If so, then close that valve and see if the problem disappears. If it does, then the problem is towards the house. If it continues (losing pressure), then suspect the well.
