My puppy was given to me at 12 weeks old. I am not sure what vaccinations she had if any. I took her immediately for her first set of shots. She had her second set of shots at 16 weeks along with her rabies shot. She is now 23 weeks old. Do I have to start the series all over again because I didn't get her vaccinated 3-4 weeks after her second set?

No, you don't have to re do the series.
Did your vet not talk to you about a vax plan for your puppy? They should have!
At 12 weeks (depending on where you got the pup) your puppy should have at least gotten 1 shot. If not, then you do the series of 3 from the moment you get the pup. Vaccines should be 3-4 weeks apart, if you don't get it on the exact date, that's OK. You're building the immune system, and each shot after the initial vaccine is just a booster. Of course, you want each vaccine done as soon as possible, but a few days up to a 1-2 weeks is OK.
Even if you waited up to a month, you can still give the shot to complete the series. But you should talk to your vet about the potency of the vaccine and the affect on your dog being so late. He will advise you of how to give the series or if you need to. I'm surprised that no one talked to about this already.
