Root canal perforation is an opening created other than the canal either due to resorption ir operators performance or due to caries

Im afraid the previous answer is ont entirely accurate. Root canal perforation usually occurs when a tooth is being root treated, and is instrumented too far. Dentists use tiny files that increase in size and length to remove necrotic nerve tissue and clean out the canal/s. These files are measured to gauge the length of the canals, and sometimes, due to the difficulty of the RCT, or difficulty in negotiating a tricky canal, the apex (tip) of the canal is perforated by the end of the file. Sometimes, it means nothing and the RCT can be completed without incident. Sometimes, it causes complications down the line, its difficult to judge. Some teeth that have been perforated, need specialist attention(endodontist)
