Her married name is correct but her maiden name is also incorrect it says Lena Angelina and the maiden name is left out. Your granddaugter's name wouldn't be on your birth certificate to begin with.

I live in New York. I was born in Brooklyn, NY and my mother was born in Italy. ( she is deceased) My granddaughter was told by the Italian consulate in NY that the corrections must be made before they can consider the application. They did not tell us where to go or how do to this.
