View Full Version : What to do to amend my sons birth certificate to get his fathers last name put on it

I live in North Carolina. My son was born in Chapel hill in 2008 and I had a daughter in 2010. My daughters birth certificate has her fathers last name but my sons does not because the lady would not except the fathers id for nothing. What do I need to do to amend my sons birth certificate and who do I need to contact?? The father signed an affidativ for my daughters birth but when my son was born she did not even offer the affidativ and they both have the same father

"The Affidavit of Parentage can be processed at the Rockingham County Public Health Department prior to the child's eighteenth (18th) birthday. You will need to make an appointment and both parties must present a valid driver's license or state-issued identification card. You may contact the Rockingham County Public Health Department at (336) 342-8215.
If a father's name is shown on the birth certificate, a court order may be necessary to change it."

The father needs to present adequate proof of his identity - a drivers license or other PHOTO id.
