I'm looking for a horror movie I've seen a long time ago. Unfortunately I haven't seen it from the beginning to remember its name.

All I remember is about a female detective or police officer, something going to an asylum to ask a convict some questions. He was tied up to a chair in a small empty room because he was considered very dangerous. Then the lights went out and he was loose.
Near the end of the movie I remember that somewhere underground there was his lair. A cave with several dead bodies floating on water.

The actor who played the villain was pretty big, bald with a round face.
He had a weird smile. I know I've seen him in other movies but I can't recall where.

Can someone ID the actor or tell me the name of this movie?
I already looked for bald actors and keywords about "cell" or "asylum" which ring some bells but no luck.

No he wasn't Anthony Hopkins. This whole scenario sounds like Hanibal but it isn't.

The actor is big sized, kind of fat but very strong. He's bald with a round face. He's pretty scary.

I'm talking about a guy probably the same size of the actor who played Raban in Dune and Hamidu in Midnight Express. Maybe even bigger that him.

A detail about the villain of this movie.

He was a silent serial killer. He wasn't talking much. Probably that's why the detective was sent, to investigate about his victim's whereabouts or to psychologically evaluate him.

Any help?
