I was with my ex for less than a year... there was never a lot of love there. When I became pregnant he bailled. I was a canadian livng and working in the USA, he made things so miseralbe that I left the state before my son was born. He has made no contact, I would like to visit the states but I am afraid that if I bring my son he or his mother might take him... and although I know in the end I would get him back... I would take some time. I want for him to give up all parental rights, as he does not pay child support shows zero interest... at the same time he cliams its because of the distance. The last time we spoke my son was 6 months old he is now 23 months. I want to know the name of the form and if I can down load it and have him sign it. Would this be legally binding.

There is no such form. Please refer to this sticky note at the top of this forum:

Only a court can terminate rights. And they will only do so in limited circumstances.

There MIGHT be help for you however. First, is the father on the birth certificate or was any court action taken to determine custody and/or paternity?
