It's transposed. I'm over 18 and married. Do I need to use my maiden name or my married name on the forms?

The form varies from State to State - if you are asking that your birth certificate be corrected you would fill out the forms in your correct BIRTH NAME.

The name on your birth certificate does not change when you marry. It is always available using your birth name.

Is that what you are asking?

My first name, Cheryl, is transposed on my birth certificate: Cherly. We moved from one state to another, and went to the DMV to apply for a new drivers license. To do so, you have to prove your identity, current ID, birth certificate, S.S. card and marriage certificate. Being that my first name is not spelled the way I've been writing it my entire life I need to correct it. I don't have the money to hire an attorney to do this, and I know I have to have a court order to do so. I just need to know when I fill out the forms, do I use my current last name to sign or do I sign using my maiden name for the petition?
