My deceased daughter was transferred to Andover Village in Ohio on july13,2005.You were her insurance company and your agent Mary?took good care of her fromSt.Vincen't ,Erie PA.(dr.h.nadworney) to Andover Village.No one can find any documentation of her ever being there.. strange because it was by extra large Ambulancefrom St.Vincent's Hospital direct to Andover,Ohio.Don't you have some payments you made to this case?She died 2/17/06. I woiukd just like to put this to rest.I followed her in my car and no one has a paper of any kind as to her ever being there or at St.Joe's hospital in Warren,ohiowhich was 29.4 miles away.I followed her when she was sentto Warren and there is no record in any fact,ambulance,hospital admission,etc.Would you please just verify that she was there and thank you for helping a very sad mom.ER

I am very sorry we can't help you with this. We are not the insurance company, nor can we verify where she was. We are a help desk, but in this instance cannot even assist you with a direction of where to look.

If you were the legal guardian then you have the right to request information and need to do so in writing from the groups.
Next while the insurance company may pay for treatment, who was there to sign paper work to admit them, what copies were you given at the office at admitance or discharge ( or death if it happened there)

OP indicates her daughter died in 06, five years ago. It would be hard to track records through a hospital for this time. I know OP needs closure for some of this, but I don't think that is possible due to circumstances of death and legal records.
I would wonder why now after all of this time, if OP followed the ambulance, then that would have been the time to receive any information that was available.
