If a third party company (insurance) requests information on medical procedure-if it was an accident or natural causes, are we within our rights not to disclose this information so rates don't increase or declined coverage.

You'll need to expand on your question. I don't know who YOU are. Are you the doctor who the insurance company is requesting information from? If you're the patient, how would you know whether it was natural causes or an accident??

Besides, if you're the patient, you need to be candid with your insurance company. Because if you're NOT, they won't raise your rates. They'll DROP you.
