I rolled over a 401k with before and after tax contirbutions to an IRA, I am 57. The 401k administrator sent the before tax contributions/earnings and the after tax earnings directly to the IRA and sent the after tax contributions to me (12k) and said no tax would be due on it. I receive two 1099-R forms, one with a G in box 7 and 0.00 in box 2a for the IRA and one with a 1 in box 7 and 0.00 in box 2a for the after tax contiributions sent to me. I would like to confirm I owe no tax/penalty on the 12k, also do I put both distributions on 1040 line 15a and 0.00 on line 15b?

You do not need to report anything on your 1040, since vboth show $0 in box 2a ("Taxable Amount"). Code "G" in box 7 indicates a direct rollover to a qualified plan, like an IRA - such rolloevers are tax free, so no worries with that first form. Code "1" on the second form means an early distribution with "no known exception" - it's basically the catch-all code if none of the other available codes apply specifically. It indicates that taxes (and possibly 10% penalty) may be due. But since box 2a is $0, you have no income to report on this and indeed do not need to report anything at all on your taxes.
