what is the slop-intercept form of the equation 4x+5y=15

Have you already done some of the calculations yourself? For those that will be able to help you the best with this, it would be helpful to know what you've already done on your own.

I guess you know how to play with the numbers so as to make the equation in the form y = mx + c where m and c are constants.
Well, make it like that. In that form, the slope is given by 'm' and the y-intercept by 'c'.
