I am not sure if this is what section my question goes under, if it is in the wrong area, please move it.

My daughter's babysitter wants to take my daughter out of town on a little trip over the weekend. I need to know what paperwork I need to give her authorization for medical treatment if there is an emergency, and verification and approval that I am aware that she has taken her out of the state. They are going an hour an a half away from where we live, but it is across the state line. I do not want her to have any problems while they are gone. Any advice would be greatly appreciated.
