Hi,
I need help filling the W-2 boxes. Here's the information I was given:
John (engineer) paricipates in his company's 401k plan, medical and FSA plans. His gross wages are $125k. From the gross salary John made pre-tax contributions of $6k to the 401k, $2.4k for health insurance for the entire family anf $1k to his medical expense FSA.

Also from John's 2008 W-2 we learn that the company reported the following:
FIT withheld $18k
Soc.Security taxes withheld $6,324
Medicare taxes withheld $1,763
State taxes withheld $5,400

I need help filling the W-2 form, particularly the following boxes (I posted my answers so far, I am not sure though):
1. Wages and tips 121600.00
2. Federal income tax withheld 18000.00
3. Social security wages 102000.00
4. Social security tax w/held 6324.00
5. Medicare wages 119724.00
6. Medicare tax withheld 1763.00
7. Social security tips -
8. Allocated tips -
9. Advance EIC payment -
10. Dependent care benefits -
11. Nonqualified plans -

I would appreciate any help. Thank you!
